Instructions For The My Story Ministry Questionnaire:

IMPORTANT – Whether you are using an Apple computer or a PC, please open the file (See link below) using Adobe Reader 10.0 or higher only. We will not be able to receive your paperwork from any other reader. Please know that Apple or Mac computers have a native pdf file reader called Apple Reader that will open the ‘My Story’ and allow you to fill it in. However, when you save it and email it to us the Apple Reader typically deletes all your hard work. To save yourself from heartbreak and a lot of extra work, please do not use Apple Reader to edit and save your information.

For Apple users:

• 1. Open the ‘My Story’ document in this email using Mozilla Firefox, Google Chrome, or another non-Apple browser.

  2. Save the ‘My Story’. Copy the exact file name. Then close that document.

  3. Open Adobe Reader. Then choose to open the document, in Adobe Reader, using the exact file name. That will open it up in Adobe Reader. You will know you have opened the document in Adobe Reader if you see the Adobe icon at the top of the page.

  4. Then go ahead and fill in the ‘My Story’ saving as you go.

  5. Open a NEW email and attach your saved ‘My Story’ found on your desktop to your email and send it to your HHN ministers or to greatgraceministries@gmail.com.

For PC users:

• 1. Click on the "Download" link below, and save the questionnaire to your computer desktop.

  2. Fill out the questionnaire, saving often while you are filling it out. Since it is saved to your desktop, you may work on it at your own pace.

  3. Once you have completed the questionnaire, please save for the final time.

  4. Open a new email and attach your saved questionnaire, found on your computer desktop, to your email and send it to your NW RTF ministers or to greatgraceministries@gmail.com.

  5. OR you can open the form below, print it out, and either scan and email, or mail the completed form to this address:
